The Clear 3-Phase Assessment Journey

To ensure a smooth and efficient assessment process, we have broken down the journey into three distinct phases. You only move to the next phase once the current requirements are met.

Phase 1: Registration and Payment (Your First Actions)
Your registration begins when you complete two crucial steps. We recommend setting aside 10-15 minutes to complete the form in one sitting.

1. Complete the Medical History Form:

• • This form gathers essential clinical data, medical history, ID, and consent.
  • It also requests details of your Nominated Familiar Person (NFP)—a key requirement for a NICE-compliant assessment.

2. Make Payment:

• • Select your preferred payment option (bank transfer or installment plan) to secure your assessment path. Payment option details will be sent to you once you submit either a Medical History Form, or submit an ‘ADHD Assessment’ inquiry via our contact form.

Once both the Form and Payment are received, you are officially registered and move to Phase 2.

Phase 2: Onboarding & Information Gathering
This phase is where we collect the final, critical data points needed for the diagnosis:

1. We will send you the ADHD Self-Assessment Form to complete.
2. We will contact your Nominated Familiar Person (NFP) with their specific form to gather their vital third-party perspective.

Phase 3: Booking Your Interview
Once all three key documents are returned (Medical History Form, Self-Assessment Form, and NFP Form), your file is complete.

A member of our team will contact you directly to offer available dates and times for your formal assessment interview with one of our specialists.
